Seems poor Shakira is upset with Madrid. Or maybe someone in Madrid?
She's decided she having a terrible time and she doesn't want to waste any more of her time in Madrid/or the Madrid dude.
She's going to pack her stuff and get out of there - far away as possible, and soon.
She thinks the place is full of wusses and add the final insult - your aniseed mouth - ugh.
It's a pretty angry song. I couldn't find if "boca de anis" was a common phrase, or just one Shakira made up.
There is a bullfighter who is clearly in love with himself; kissing himself, posing, laughing egotistically (maybe part of the job description?) who is playing Shakira's lover.
She alternatively in bed, in a dressing room dressed like a bullfighter/bull or running away from Madrid with a massive pair of sissors.
She's kinda acting like a bull in a fight being attacked by the bullfighter, then maybe goes to kiss him, but just rubs her hands on his mouth as a rejection.
In the end she cuts off big chunks of her own hair with the scissors she's been lumping around.
